Killed,revived and still lying there (dead) or alive cant even tell.
How its possible after so many years problems like this?
Solution shutdown game and restart...

Happens so rare I even forgot it exists, every game has its bugs, get over it!

You are alive, its a graphical issue, but your avatar standing revived.
If you could move to mobs and agro them again to kill you, the next revive would solve the issue without the need of restart the game.
Problem is, mostly the revive is not even close, and you wont see your "alive" avatar moving as the camera still shows the dead one.

It is a solution on mayhem maps, where it is relative easy to move your invisible "alive" avatar to the mobs without seeing it, to get killed again and revived.
