I think the rare ones are supposed to drop from the mobs as well, however that never happened to me so can't confirm.The rare token costs are so odd. It seems you need to open boxes to get rare tokens and then grind normal tokens during mm? At least thats how it looks on EL?

It seems Rare Mayhem Token cost is related to the name modifier of the item.
Because of this the incredibly powerful Augmented Azuro costs 3x Rare Tokens for a whooping 4.7 dps. Ahem.... cough cough. Makes sense.
- None or Adjusted: 1 Rare Token
- Improved or Modified: 2 Rare Tokens
- Augmented: 3 Rare Tokens
- Perfected: 4 Rare Tokens
