I've had a few occasions, the most recent flying to Setesh from Caly, where I've received the "you have entered lootable pvp" notification, but the lootable skull icon was still yellow (the pvp one was red). I did receive the exit notice shortly after, so I'm wondering if it was just a tiny patch of lootable space.

Should I trust the icon over the notice?

From my understanding the issue is that the borders changed but the messages were not.

Pay attention to the symbols around the radar, I always worry about hitting a spot without the message

It's crazy that it has been like this for a decade or something but it's a known bug. I wonder why MA keeps it ... Probably scares off anyone going to a new planet for the first time then see that scary message that they can lose everything and not sure what is actually lootable and whats not, where it begins and where it ends, if an easy to fix message is bugged then what else is bugged, and turn right around and park on caly for eternity lol

there is squere 1x1 m at Toulan planet where skull is red = lootable

Scroll down that post and she has a decent image of the lootable zones.

It's really crazy the community just got used to a number of bugs and just take them as they are and stopped pointing them out.... it's like if they don't complain too much, why invest resources to fix a bug, right??? Some bugs are a decade old... how much longer does MA need to start addressing common sense shit in the game??

IF mindark is still working on unreal engine 5 and IF they actually release it and IF unreal makes it much easier to fix bugs and code in general ... then I really hope they got a list they can run down on all the bugs and bang out a million fixes asap .. but lot of IFs in there lol. There's probably been so many people we lost who tried out the game and quit immediately upon seeing bugs left and right. or people who would have played watched a streamer come across bugs everywhere and said "nah I'm not downloading this bug infested game" ... its definitely important to not ignore them forever

Having survived 2 game engine changes I can tell you with certainty that there's going to be plenty of bugs for the rest of time, and new game engine may be the root of that problem likely. Lets just say that there's a good reason my avatar does not currently own any estates. Game engine changes and all related bugs and unusable items and estates (some of which were unusable for over 2 years) play a major part in that.
